Job Description

Our Community Living Support program provides one on one support to individuals living with mental health challenges. We focus on the direct needs of individuals and support them realizing their full potential and achieving independence. In this full time (35 hours per week) position, our Community Support Worker will be responsible for supporting clients with severe and persistent mental health issues to live independently within the community. The majority of the work is done with clients in their home and/or within the community.

QUALIFICATIONS

  • Minimum of a two-year diploma or degree in a social services related field.
  • Minimum of one year of relevant experience working with adults living with mental health issues.
  • Knowledge of the principles of psycho-social rehabilitation.

Additional Requirements

  • Clear Criminal Record Check is required.
  • Current First Aid and CPR Certificate.
  • Valid and unrestricted Class 5 BC Driver’s License with access to a safe and reliable vehicle.
  • Position requires Union Membership.
  • Shift/Schedule/Hours per week: 35 hours per week
  • Expected Start Date: ASAP
  • Wage/Salary: Starting at $27.54/hour

ACCOUNTABILITIES

  • Facilitating life skills training that includes teaching of self-care, home management, community management and social leisure skills.
  • Assisting clients in gaining necessary knowledge of available services within the community, how to access services independently, and how to advocate for self.
  • Assist client in gaining the skills necessary to successfully acquire appropriate living accommodations.
  • Providing assistance navigating a variety of environments such as home, grocery stores, banks, etc.
  • Providing updates to the clients’ Primary Therapist to fulfill case conferencing expectations as per the Individual Services Plan Agreement.
  • Advocating/liaising on behalf of the client to deal with any concerns/issues at hand, as required.
  • Supporting clients by referring pertinent issues to the client’s Primary Therapist at Mental Health, (e.g. medication, suicidal ideations, family concerns, changes in client’s functioning level).
